TIME:  8:30 A. M. – 5:00 P.M.       

 

Job Summary:  Access the Electronic Health Record as needed to scan appropriate documents into the electronic record, responds to telephone requests, and assists in general office activities as required.  Utilizes the Electronic Health record as appropriate to fulfill any patient, staff and physician requests. 

 

Summary of Accountabilities:

 

  1. Accesses the electronic health record for visits, correspondence, testing, etc., making sure all up to date information scanned into the EHR.
  2. Responsible for mail delivery, pick up, open and completion.
  3. Routine Daily retrieval of documents from departments (Family Medicine/OB/GYN Peds)
  4. Scan all patient medical records for departments (Family Medicine/OB/GYN/Ped)
  5. Task all pertinent information to providers.
  6. Capture from fax and or mail all provider correspondence and results and forward to appropriate provider for review and signature.
  7. Maintain up-to-date records for patient release of information authorizations.
  8. Perform release of information requests for patients, hospitals, physician offices, and other requestors following the standard authorization and identification validation process. (AKA- Correspondence)
  9. Detail Attention to Attorneys’ request including Subpoena’s.
  10. Process records requests from hospitals, physicians’ offices, patients, Social Security, Disability, legal and insurance, and determine fee for same.
  11. Maintain chart supplies for the office.
  12. Perform other clerical duties which may be assigned or assumed as time as needed
  13. Print Copies of Immunization Records for Osborn Patients
  14. RED BOOK- Daily update of the Red Book  - Follow-up with GYN incoming transfer records

 

Minimum Certifications, Registry and/or Licenses Required: 

 

Minimum Degree Required and Experience Required:  High School diploma or GED equivalent required.  Minimum three years experience as a Medical Record Clerk preferred.

 

 

 

Skill Requirements:

  1. Must have strong interpersonal, oral, and written communication skills with the ability to communicate effectively with patients, physicians, staff, Attorneys, and outside vendors
  2. Must be customer focused and results oriented.
  3. Good command of the English language, both oral and written.
  4. Strong organizational skills required.
  5. Possess the ability to handle confidential information as appropriate and in compliance with HIPAA regulations.
  6. Ability to use all applicable office equipment such as fax machine, copier, telephone, and scanner.
  7. Basic computer skills required.  Advanced computer skills preferred.
  8. Previous Electronic and Medical Record experience preferred.
  9. Medical Terminology knowledge preferred.

Environmental Conditions: Environmental Conditions:

Interior office setting.  The worker is subject to inside environmental conditions.
