ASSIGNED DUTIES
|

DESCRIPTION OF WORK DUTIES AND RESPONSIBILITIES: ( Type in or attach current

- The peers' main roles will be to serve as recovery coaches, as well as navigators who specialize in providing guidance for accessing community services. Referrals can be accepted from anyone, but the primary goal of this service is 'first responders' such as EMS, law enforcement and medical providers to refer to the team. Ideally, one of the team members would engage with the survivor in the ED, and then follow up continues with the team for up to 90?days post-ED discharge. CORE's primary focus will be linking overdose survivors to recovery support and treatment services post-overdose
- Provide Support and education about the recovery process to families, friends, allies and others
- Provide Intervention at local ECs, Recovery Centers or in the community when an individual has OD'd and been referred by EMS or other First Responder
- Complement and coordinate service referral and intake processes to individuals in need of AD services at Aspire BHDD
- Work with individuals identified as being at high risk of return to detention and individuals with SUD.
- Incorporates outreach and engagement activities to build rapport and trust among the target population
- Provides training of basic personal and social skills, simple academics, or work skills to individuals/students in a state hospital, juvenile detention facility, residence, group home, workshop, or supported employment environment.
- Instructs and advises clients/residents on daily living skills in one-on-one or group instruction. May provide training on behavioral problems, daily decision making, and the resolution of minor problem.
- Provide support to individuals and families during periods of incapacitation, family disruption or convalescence, providing companionship, personal care, and help in adjusting to new lifestyles. Arranges for needed medical care and provides needed services to assure that medical care is obtained. Assists in presentation of educational group lectures related to appropriate topics.
- Link peers up with various community organizations and resources to provide additional support
- Attends agency required training, meeting as scheduled by supervisor.
- Performs other duties as assigned, not to exceed 90 days, as assigned by supervisor.
