B. QUALIFICATIONS
Bachelor's degree in a social services related field AND one year experience in a social services related position OR A high school diploma (or GED) AND three years of full time experience in social services delivery (e.g. case management, outreach, prevention/education, etc). A Human Services Bachelor degree is preferred.
Possess basic reading, writing, and math skills; able to lift 50 pounds; able to complete Crisis Intervention training; able to use a computer; able to maintain a valid driver's license and able maintain valid auto insurance.

G. AGE RELATED AND SPECIAL NEEDS OF POPULATION SERVED
Work requires the knowledge and skills necessary to provide direct individual services appropriate to Developmental Disabilities whose ages range from 18 to retirement.
Staff should be able to:
a. Demonstrate knowledge and skill in non-violent crisis intervention techniques.
b. Display knowledge of agency, community, and institutional resources and referral processes.
c. Assist individuals and staff in appropriate goal setting and implementation of the treatment plan; display knowledge of discharge planning
d. Display knowledge of HFR, Regional Board, CARF, and AACSB compliance requirements.
H. ASSIGNED DUTIES
DESCRIPTION OF WORK DUTIES AND RESPONSIBILITIES: The agency expects employees to perform their work in a competent and efficient manner. Under limited supervision, Care Plan Coordinator ensures all day services are person centered and provided in accordance with agency policies and procedure. The primary work location is the service center.
Duties Include:
1. Maintain individual medical records
- Attend staffings and participate in goal planning
- Prepare/monitor reports and assessments such as the Bio-psychosocial, HRST, Integrated Summary, AIMS, etc.
- Attend training required of all direct staff members
- Supervise staff.
- This includes, but is not limited to, scheduling work hours, completing MRFs and PMFs, completing or reviewing incident reports, and scheduling and monitoring training to ensure all ar e completed in accordance with policy.
- Counsel employees to ensure they know what is working and what is not working.
- Review tracking notes for accuracy (following the intent of the goals) and detail. Monitor tracking for correct frequency.
- Complete 2 face to face visits per month with each individual on your caseload.
- Complete contact notes monthly
- Provide guidance and support to instructors on the development and implementation of activities (curriculum).
- Provide guidance and support to instructors unable to manage inappropriate behaviors.
- May be required to transport individuals using your personal vehicle or an agency vehicle.
- May be required to cover a classroom when required by staffing ratios.
- Conduct quarterly supervisory visits with those who report directly to you
- Serve as the primary supervisor for transportation, CAI, ?supported employment.
- Other duties are required.
Normal hours: Monday-Friday, 8:00 AM – 4:00 PM. Position does require flexibility to work various day, evening, night and weekend shifts when needed. May be necessary to participate in emergency call rotation or cover shift if adequate coverage cannot be obtained by direct care staff. This is an hourly position and deemed non-exempt.
