Disability Determination Specialist

This position is responsible for processing medical records requests ensuring the security and confidentiality of records.

 

DESCRIPTION OF DUTIES:

Verify and update client demographic information in the Electronic Health Record (HER) system.

Maintain medical records and process requests for records.

Respond to requests for medical information from the Social Security Administration, attorneys, or consumers within 72 hours of receipt.

Process subpoenas and court orders for medical records.

Validate and track all requests and release of information for medical records.

Electronically process medical records and return to the appropriate agencies, attorneys, or clients.

Accurately, thoroughly, and timely process all requests for records.

Scans/ Imports documents and medical records into the EHR system.

Retrieve medical records/ documents from hospitals or other outside agencies for scheduled appointments.

Compares EHR data with resource documents to detect errors and initiate corrective action.

Assists with pulling and filing electronic records and paper charts as needed.

Assist with purging of inactive paper charts according to the company’s retention policy.

Maintain client confidentiality by adhering to HIPAA regulations and/or company policies.

Assist with State Reporting data corrections.

Provide backup for front desk and call center coverage at Carastar and all its’ Satellite offices as needed.

Answer telephone calls, take messages and return calls.

Perform other related duties as requested by supervisor.

 

REQUIREMENTS:

Ability to work independently and perform clerical assignments with moderate direction.

Effective written and verbal communication.

Ability to meet deadlines.

Ability to follow oral and written instructions.

Basic computer literacy.

Proficiency in Microsoft Excel.

Strong attention to detail with excellent organizational skills.

Ability to sit for prolonged periods of time.

Ability to lift or move up to twenty-five (25) pounds.

Must have reliable transportation and be willing to travel among all Carastar facilities.

 

QUALIFICATIONS:

High diploma or GED from an accredited institution, plus five (5) years of medical experience. Associate degree or higher in appropriate field preferred

Must hold and maintain a valid driver’s license and have reliable transportation.

Must have and maintain a driving record that is acceptable to Carastar’s insurance carrier.

Must maintain at least liability coverage on personal vehicles.
