About the Role

The Intake Coordinator plays a vital role under the general direction of the Operations Supervisor or their designee. This position involves various responsibilities, including consumer verification, intakes, home visitations, and record management.
 
What You Will Do
  • Receive, review, enter, and verify all intakes.
  • Maintain ongoing communication (verbal and written) with service supervisors, case workers, consumers, and other departments.
  • Ensure contracts are current and evaluation packets are updated annually, with necessary documents signed.
  • Handle consumer terminations per established practices and maintain an active consumer census.
  • Accurately enter data related to contracts, terminations, and consumer changes in accordance with company policies.
  • Input pertinent consumer paperwork into the appropriate respite system in a timely manner.
  • Conduct home visitations and evaluations.
  • Ensure consumers are assigned appropriate care providers based on family needs.
  • Follow up on unreported respite hours and compile monthly reports.
  • Provide excellent customer service and accurate information.
  • Assist in other administrative tasks as needed.
  • Support quarterly care provider meetings and attend any vendor meetings.
  • Attend mandatory meetings punctually.

What You Need

  • High school diploma or GED; Associate's Degree in Social Work or related field preferred (2 years of experience may substitute for 1 year of post-secondary education).
  • Bilingual in English and Spanish is required.
  • Proficient in basic business arithmetic with a typing speed of 30 wpm.
  • Familiarity with developmental services and excellent customer service skills.
  • Strong organizational, time-management, and problem-solving abilities.
  • Proficient in MS Excel, Outlook, PowerPoint, Word, and Teams.
  • Experience with cloud-based e-signature solutions such as HelloSign or DocuSign.
  • Valid California Driver's License and auto insurance.
