  1. ASSIGNED DUTIES

DESCRIPTION OF WORK DUTIES AND RESPONSIBILITIES: (Type in or attach current description of duties.  Employees are expected to perform their work in a competent and efficient manner.  Include % of time).

 

Duties Include:

Ability to provide care coordination services to individuals with behavioral health diagnosis who have been admitted to the local hospital.  Ability to collaborate with hospital personnel for the purpose of shared information gathering, providing care plans and supports to individuals enrolled in care and assistance with connecting individuals who need to be referred for care. Ability to abide by agency and state standards around service delivery and documentation of services. Ability to provide life skill teaching and linkage to valuable resources as well as connecting individuals to natural supports that are in line with individual goals. Employees should complete weekly case management spreadsheets, ensure that other requirements for care are up to date, participate in agency, state and other trainings, participation on stakeholder outreach and collaborative events. Employee should also participate in multidisciplinary meetings, staff meetings, and agency related meetings.  Other duties as assigned.
