B. QUALIFICATIONS:
Knowledge of agency policies and procedures. Ability to coordinate and organize delivery services. Ability to monitor consumer's progress toward meeting established goals. Knowledge of consumer's treatment goals. Ability to interview consumers and or families using established techniques. Ability to develop consumer profile. Knowledge of agency confidentiality policies. Knowledge of state and federal confidentiality laws and regulations. Knowledge of available community resources. Ability to make appropriate referrals. Knowledge of crisis intervention. Ability to develop client service plan to habilitate and rehabilitate client and assist client in attaining social, educational, and vocational goals. Ability to assist with individual counseling sessions. Knowledge of counseling techniques. Ability to contact health care professionals to obtain additional background information. Ability to develop, implement, and facilitate workshops. Knowledge of target population. Ability to access what training is needed. Knowledge of agency specific software. Knowledge of available databases. Ability to prepare reports and case history records. Knowledge of eligibility requirements. Knowledge of information needed to complete applications. Knowledge of what qualifies as an emergency situation. Ability to delegate assignments. Ability to evaluate, mentor and motivate residents. Ability to review the work of others. Ability to work independently. Ability to answer resident questions. Ability to serve as a subject matter expert to the unit. Entry qualifications include a high school diploma or GED or higher, who have completed the paraprofessional training required for DBHDD contracted organizations and under the supervision of a Residential Manager/Supervisor may perform residential services.

provide consistent, high-quality, consumer focused services.
G. ASSIGNED DUTIES
Provides a broad range of paraprofessional social services to patients/ clients/ consumers and their families.
Conducts patient/client/consumer intakes, participates in treatment team planning, and performs crisis intervention. Assist in dealing with personal and social problems. May provide supportive counseling to consumers and families and/or serve as a liaison for social services. May perform case management duties. Performs lead worker functions.
Duties Include:
Implements and organizes the delivery of specific social services within the community. Monitors patient's/client's/consumer's progress towards treatment goals. Completes patient/client/consumer intakes based on interviews with patients/clients/consumers, their families, significant others and appropriate community agencies. Assists clients in locating and utilizing community resources including legal, medical, financial assistance, and other referral services. Provides short term case management and referral services to clients with emergency situations. Provides crisis intervention and emergency shelter services. Counsels clients living in the apartments; assists in prerelease and release planning. Maintains contact with other social service agencies and health care providers involved with clients to provide information and obtain feedback on client's overall progress. Implements life skills workshops and programs in behavior management, adult services, community and social services. Maintains program statistics for purposes of evaluation and research. Prepares intake reports and case history records. Performs lead worker functions (comprehensive needs assessments, housing goals, primary & Secondary/Contingency Transition Plan, Residential Crisis Response Plan and Residential Functional Assessment within the first 7 days of admission and every 90 days to determine appropriateness for this level of residential support. Performs job responsibilities with minimal supervision. Serves as job expert or organization resource in assigned areas.
NOTE: This position will answer directly to the Residential Coordinator
